United Elite Dental Care ("UEDC," "we," "us," or "our") provides dental benefit plans to individuals and families in Florida. This Privacy Policy explains what personal information we collect when you visit unitedelitedentalcare.com, enroll in a plan, use your member account, or communicate with us, how we use and share that information, and the choices you have. By using our website or services you agree to the practices described here.
When you enroll in a plan we collect the information needed to set up and administer your coverage: your name, date of birth, gender, home address, email address, mobile phone number and any additional phone numbers you provide, the plan you select, your preferred language, and the same details for any dependents you add. Payment card information is entered directly into a secure form provided by our payment processor, Square; we receive only a payment token, the card brand, and the last four digits, never your full card number. If you were referred by a licensed agent or enrolled through our call center, we record the agent or agency associated with your enrollment. When you contact us through the website we collect the name, contact details, and message you submit. We also collect standard technical information such as your IP address, browser type, and the pages you visit.
We use your information to enroll you in the plan you selected and confirm your eligibility for benefits; to bill your recurring monthly premium and send you receipts, payment reminders, and notices about the status of your plan; to create and secure your online member account, including sending you a link to set up your password and verifying your identity when you sign in; to respond to your questions and requests; to pay commissions to the agent who assisted you; to prevent fraud and duplicate enrollments; to meet our legal, regulatory, and accounting obligations; and to improve our website and services.
If you check the optional text-message consent box on our enrollment form, United Elite Dental Care will send account and service messages to the mobile number you provide, such as a link to set up your member account, payment confirmations, failed-payment alerts, and notices that your plan has been suspended or reinstated. Consent is not a condition of purchase. Message frequency varies, typically one to four messages per month. Message and data rates may apply. You can withdraw your consent at any time by replying STOP to any message, and you can reply HELP or call 800-572-3545 for assistance. We record the date, time, source, and IP address of your opt-in as proof of consent. Full program details are in our Mobile Messaging Terms, linked below.
We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes. Mobile opt-in information is used only to deliver the messages you agreed to receive.
We share your information only as needed to provide your dental plan. Your enrollment and eligibility details are transmitted to Florida Dental Benefits, Inc., the licensed Prepaid Limited Health Services Organization that administers the dental benefits, so that your participating dentist can confirm your coverage. Payment information is processed by Square, Inc. under its own privacy policy. We use service providers to send email and text messages, host our systems, and deliver website chat and mapping features; these providers may process your information only on our behalf and under our instructions. The licensed agent or agency that assisted with your enrollment can view the enrollment they submitted. We may also disclose information when required by law, subpoena, or regulatory request, to protect our rights or the safety of others, or in connection with a merger, acquisition, or sale of assets. We do not sell your personal information.
Our website uses a small number of cookies and similar technologies. A language cookie remembers whether you chose English or Spanish. Our enrollment form stores an in-progress application so you can return to finish it. Our website chat is provided by Tawk.to and our Find a Dentist map is provided by Google Maps; each may set its own cookies and collect usage data under its own privacy policy. You can control cookies through your browser settings, but some features of the website may not work correctly if cookies are disabled. We do not use cookies for cross-site advertising.
We use administrative, technical, and physical safeguards designed to protect your personal information, including encryption of data in transit, restricted access to our systems, and secure storage of account credentials. Payment cards are never stored on our servers. Links we send to set up or reset your account are single-use and expire after 72 hours. No method of transmission or storage is completely secure, so we cannot guarantee absolute security, but we work to protect your information in a manner consistent with industry practice and applicable law.
We keep your enrollment, billing, and communication records for as long as you are a member and afterward for the period required by Florida insurance regulations, tax and accounting rules, and our legitimate business needs, such as resolving disputes and enforcing our agreements. Incomplete enrollment applications that are never paid are removed after a short period. Records of your text-message consent and opt-out requests are retained as proof of compliance.
You can update your contact information or payment method at any time by signing in to your member account or calling Member Services. You can stop text messages by replying STOP to any message. You can ask us what personal information we hold about you, request a correction, or request deletion of information we are not required to keep by contacting us using the details below; we will verify your identity before acting on a request. Because some information is needed to administer your plan and to meet our legal obligations, certain requests may be limited while your coverage is active.
We may update this Privacy Policy from time to time. When we do, we will post the revised version on this page and update the effective date above. Continued use of our website or services after a change means you accept the updated policy. If you have questions about this policy or about how United Elite Dental Care handles your information, call Member Services at 800-572-3545 or use the contact form on our website. Our services are intended for residents of Florida; children under 18 may be enrolled only by a parent or legal guardian, and we do not knowingly collect information directly from children.
