1. Acceptance of These Terms

You accept these Terms when you access or use our website, complete an enrollment yourself or through a licensed agent or UEDC representative, make a premium payment, or sign in to your member account. If you are enrolling on behalf of your spouse, a dependent, or a minor child as their parent or legal guardian, you represent that you have the authority to accept these Terms on their behalf. If you do not agree to these Terms, please do not use our website or enroll in a plan.

2. About United Elite Dental Care and Our Plans

United Elite Dental Care markets and enrolls members in prepaid dental plans available to residents of Florida. The dental benefits are administered by Florida Dental Benefits, Inc., which is licensed pursuant to Part I, Chapter 636, Florida Statutes, as a Prepaid Limited Health Services Organization. The specific services covered, member co-payments, limitations, and exclusions are described in the plan documents and schedule of benefits you receive at enrollment. If anything in these Terms conflicts with your plan documents, the plan documents control with respect to your dental benefits.

3. Eligibility and Enrollment

To enroll as the primary member you must be at least 18 years old and a resident of Florida. You may add your spouse and eligible dependents to your plan, and a parent or legal guardian may enroll a minor child as the responsible party. You agree to provide accurate, current, and complete information for yourself and each person you enroll, and to update that information promptly if it changes. Enrollments may be completed on our website, by phone with a UEDC call-center representative, or with a licensed agent. Agents are independent and are compensated by UEDC; they are not authorized to change these Terms or your plan documents. Your coverage begins on the effective date shown in your enrollment confirmation, after your first premium payment is received. We may decline, cancel, or correct an enrollment that contains false or duplicate information.

4. Premiums, Billing, and Recurring Payments

Plan premiums are billed monthly. When you enroll, your payment card is charged for your first month's premium, and by accepting the Recurring Payment Authorization on the enrollment form you authorize UEDC to charge the card on file automatically each month for as long as your plan remains active. Payments are processed by Square, Inc.; UEDC does not store your full card number. You are responsible for keeping a valid payment method on file and can update it at any time through your member account or by calling Member Services. If a payment cannot be processed we will notify you and attempt the charge again; if the premium remains unpaid your plan may be suspended and, after continued non-payment, terminated. A suspended plan is reinstated when the past-due premium is paid. We may change premium rates for future billing periods with advance notice to you as required by law and your plan documents.

5. Cancellation, Termination, and Refunds

You may cancel your plan at any time by calling Member Services at 800-572-3545 or through your member account. Cancellation stops future recurring charges; coverage continues through the end of the period for which you have already paid, unless your plan documents provide otherwise. Refunds, if any, are governed by your plan documents and applicable Florida law. UEDC may terminate your plan for non-payment of premium, for fraud or material misrepresentation in your enrollment, for misuse of benefits, or if you no longer meet the eligibility requirements. Termination does not relieve you of premiums owed for periods in which you were covered.

6. Your Member Account

After you enroll we send you a secure, single-use link to set up your online member account, where you can view your plan, update your contact and payment information, and manage your preferences. You are responsible for keeping your sign-in credentials confidential and for all activity that occurs under your account. Notify us immediately at 800-572-3545 if you believe your account has been accessed without authorization. We may suspend or disable an account to protect you, other members, or our systems.

7. Mobile Messaging (SMS) Program

This section is the Mobile Messaging Terms for the United Elite Dental Care text-message program. By checking the text-message consent box on our enrollment form, or by giving verbal consent to a UEDC representative or licensed agent who enrolls you by phone, you agree to the terms in this section.

Program description

The United Elite Dental Care account and service messaging program sends transactional text messages about your dental plan to the mobile number you provide at enrollment. Messages may include a secure link to set up your online member account, confirmation that a monthly premium payment was received, an alert that a payment could not be processed and how to update your card on file, a notice that your plan has been suspended for non-payment, and confirmation that your plan has been reinstated. This program does not send marketing or promotional messages.

How you opt in

You opt in by checking the separate, optional "Text message updates" box on the United Elite Dental Care enrollment form at unitedelitedentalcare.com/enrollments, or on the same form when a licensed agent or UEDC call-center representative completes your enrollment with you. The box is never pre-checked. When a representative enrolls you by phone, they read the consent statement aloud and check the box only after you say yes. Consent to receive text messages is not a condition of purchasing a plan or of receiving any UEDC service. We record the date, time, source, and IP address of each opt-in.

Message frequency

Message frequency varies based on activity on your account. Most members receive one message when they enroll and zero to two messages per month afterward, tied to billing events. In a month with a failed payment, plan suspension, and reinstatement you may receive up to four messages. Messages are sent only in response to events on your own account and are never sent on a recurring schedule for promotional purposes.

Message and data rates

Message and data rates may apply to each text message sent or received under this program, according to your mobile carrier's plan. United Elite Dental Care does not charge for text messages, but you are responsible for any charges from your wireless carrier. Contact your carrier for details about your messaging plan.

How to opt out (STOP)

You can cancel text messages at any time by replying STOP to any message you receive from us. After you reply STOP we will send one final message confirming that you have been unsubscribed, and you will receive no further text messages from this program. You may also opt out by calling Member Services at 800-572-3545. Opting out of text messages does not affect your dental plan coverage or your ability to receive account notices by email. If you later wish to receive texts again, contact Member Services and we will ask you to confirm your consent.

How to get help (HELP)

For help with this program, reply HELP to any message and you will receive a message with our contact information, or call Member Services at 800-572-3545 (Monday through Friday during business hours, Eastern Time). You can also reach us through the contact form at unitedelitedentalcare.com.

Supported carriers and carrier liability

This program is available on major U.S. wireless carriers, including AT&T, T-Mobile, Verizon, and their affiliated networks. Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your carrier and network availability, and we are not responsible for messages that are lost, delayed, or misdirected because of carrier issues or an incorrect or changed mobile number. You represent that you are the account holder or an authorized user of the mobile number you provide and that you will notify us if that number changes.

Privacy of your mobile information

Your mobile number and opt-in information are used only to deliver the messages described in this section. We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes. Text messages are delivered through a messaging service provider that processes your number solely on our behalf. For details on how we collect, use, and protect your information, see our Privacy Policy.

Account and service notices are sent by email to the address on file; these are transactional messages required to administer your plan and are sent regardless of your text-message preference. You are responsible for keeping your contact information current. Emails and messages we send may include links to our website; we will never ask you to provide your full payment card number by email or text.

9. Acceptable Use of the Website

You agree to use our website and member account only for lawful purposes related to learning about, enrolling in, and managing a United Elite Dental Care plan. You may not submit false or misleading information; enroll a person without their authorization; attempt to access another member's account or our systems without permission; interfere with the operation or security of the website; use automated tools to scrape, harvest, or submit data; or use our contact and enrollment forms to send unsolicited commercial messages. We may block or restrict access by anyone who violates these rules.

10. Intellectual Property

The content of our website, including text, graphics, logos, images, page design, and software, is owned by or licensed to United Elite Dental Care and is protected by copyright, trademark, and other laws. "United Elite Dental Care" and our logo are our trademarks. You may view and print pages for your personal, non-commercial use in connection with your plan. You may not reproduce, distribute, modify, or create derivative works from our content, or use our name or logo, without our prior written permission.

11. Participating Dentists and Third-Party Services

Dentists who accept our plans are independent licensed professionals; they are not employees or agents of United Elite Dental Care, and UEDC does not provide dental care or control the treatment decisions of any dentist. Our Find a Dentist directory is provided for convenience and is updated periodically; please confirm participation directly with the dental office before your visit. Our website uses third-party services such as Square for payment processing, Google Maps for the provider directory, and Tawk.to for website chat, and may link to other websites. These services and websites are governed by their own terms and privacy policies, and we are not responsible for their content or practices.

12. Disclaimers

Information on our website is provided for general informational purposes and is not dental or medical advice; always consult a licensed dentist about your oral health. We work to keep the website accurate and available, but it is provided "as is" and "as available" without warranties of any kind, whether express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the website will be uninterrupted, error-free, or free of harmful components. Nothing in this section limits the benefits you are entitled to under your plan documents or any rights you have under Florida law that cannot be waived.

13. Limitation of Liability

To the fullest extent permitted by law, United Elite Dental Care and its officers, employees, agents, and service providers will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of data or profits, arising out of or related to your use of the website or these Terms, even if we have been advised of the possibility of such damages. Our total liability to you for any claim relating to the website or these Terms will not exceed the premiums you paid to UEDC in the twelve months before the event giving rise to the claim. This limitation does not apply to the dental benefits provided under your plan documents, to liability that cannot be limited under applicable law, or to claims arising from our gross negligence or willful misconduct.

14. Indemnification

You agree to indemnify and hold harmless United Elite Dental Care and its officers, employees, and agents from any claims, losses, liabilities, and expenses, including reasonable attorneys' fees, that arise from your violation of these Terms, your submission of false or unauthorized information, or your misuse of the website or your member account.

15. Governing Law and Disputes

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-law rules. If you have a concern, please contact Member Services first so that we can try to resolve it informally. Any dispute that cannot be resolved informally will be brought in the state or federal courts located in Florida, and you consent to the jurisdiction of those courts. Questions or complaints about a Prepaid Limited Health Services Organization may also be directed to the Florida Department of Financial Services, Division of Consumer Services. If any provision of these Terms is found unenforceable, the remaining provisions remain in effect. Our failure to enforce a provision is not a waiver of our right to do so later.

16. Changes to These Terms

We may update these Terms from time to time. When we do, we will post the revised Terms on this page and update the effective date shown above. If we make a material change that affects active members, such as a change to billing practices or to the types of text messages you receive, we will notify you by email or text before it takes effect. Your continued use of the website or your plan after a change means you accept the updated Terms.

17. Contact Us
